“Loaded” describes the Evidence Engine, not the entire external data universe. Every update must preserve source identity, reporting period, denominator, and review status.
Check the official catalog; ingest only a final or clearly labeled release and preserve its checksum.
Check for a final 2025 file; retain revised-final labels and never overwrite an earlier source silently.
Reconcile records where staffed beds exceed licensed beds or where campuses and non-acute facilities share a reporting scope.
Extend the same-quarter comparison to the remaining reviewed hospitals before presenting it as statewide coverage.
Recheck ownership, license, campus, closure, and consolidation records whenever a material service change is detected.
Keep each source date attached to its measure and do not combine mixed periods into a single current-state score.
A refresh is complete only after facility identities, campus scope, source definitions, denominators, revisions, and comparability are checked. CNO will not silently substitute a newer number into an older analytical frame.
Freshness is a documented chain of custody—not merely a recent date.
