    Findings & sources Β· 10

Patient-protection findings add to workforce, financial and rebuilding questions

Oakland pediatric campus at 747 52nd Street: HCAI 106010776, CMS 053301 and CDPH ELMS 140000014. The 2025 capacity feed displays 163 beds, while the July 2024 planning baseline separately names 177 licensed, 163 operational and 141 functional beds. The retrieved 2025 source field still needs a terminology check before it is treated as any one of those concepts. Financial years end June 30; earlier figures precede the 2025 UC employment transition.

Newer patient-protection findings need to be read with their resolutions

CMS complaint surveys document patient-protection and nursing-supervision failures in July 2025 and April 2026. The July narrative records Immediate Jeopardy lifted August 5, 2025 after onsite verification; the amended April narrative records jeopardy lifted May 14, 2026 after an acceptable action plan. A separate November 2025 report describes a physician striking a restrained patient and records a summary suspension and internal investigation. These are dated findings, not a claim of ongoing jeopardy. The correction plans and later practice remain to be assessed.

Reported RN labor rose despite fewer patient days

Between FY2021–22 and FY2023–24, productive RN hours rose 17.8% to 901,368 while patient days fell 1.4% to 39,922. Nursing FTEs rose from 429 to 503. RN hours per patient day increased from 18.91 to 22.58, with a lower 2023 value of 18.72. Those are pediatric-hospital accounting figures; the relevant comparison is with pediatric peers and the hospital’s own service mix.

    Inspect evidence 1 β†’Inspect evidence 2 β†’

Licensed beds overstate what could actually be used

The July 2024 Regents proposal distinguishes 177 licensed, 163 operational and 141 functional beds in its planning baseline. Shared rooms and age, sex and infection cohorting limited usable capacity. The proposed finished campus has 208 beds, including 20 new inpatient psychiatric beds. Its NICU plan reduces the count from 55 to 36 but replaces constrained wards with primarily private rooms. A lower nominal NICU count therefore does not, on its own, establish a loss of usable care capacity.

    Source β†’

A matching 163 is not yet a verified capacity definition

The 2025 capacity feed also displays 163 beds, the same number the July 2024 Regents proposal calls operational. That numerical match does not establish that the two records measure the same thing: the retrieved 2025 source set has not yet verified whether its field means licensed, staffed, operational, pediatric-only, or another capacity concept. Until that field is checked against its original source definition, it is a reported 2025 capacity valueβ€”not a substitute for the planning document’s licensed/operational/functional distinction.

    Inspect evidence 1 β†’Source 2 β†’

The rebuild has a current milestone and a future target

UCSF’s current project page gives 2031 as expected completion. Temescal Hall was completed in September 2026; it is administrative space, with no clinical functions. The new hospital project includes expanded ED and surgical services, private patient rooms and inpatient behavioral health. The capital project is future capacity, not additional staffed beds available today.

    Source β†’

Newer quarterly finances need an accounting bridge

FY2023–24 reported $58.5 million operating income and $96.8 million net income. The newer quarterly series is sharply different: four of five quarters show calculated operating losses, while Q2 2025 shows a $258.0 million surplus with both a revenue spike and much lower reported expenses. Q1 2026 shows a $58.1 million operating loss. Reconcile these as-filed accounts with transition-related transactions and cost allocations before treating them as a stable financial trend.

    Inspect evidence 1 β†’Inspect evidence 2 β†’

    The workforce transition was contested

UCSF announced that more than 2,800 Oakland staff would transition to UC employment in July 2025. NUHW’s strike-end statement describes approximately 1,300 represented workers disputing contract cancellation, benefits costs and representation; its authorized striking groups included nursing assistants, respiratory therapists and other support workers. Those are attributed labor positions, distinct from the earlier RN accounting series. Current retention and support-staff continuity are important follow-up records.

Adult federal measures leave a pediatric evidence gap

The matched CMS hospital record marks its loaded nurse-experience, infection, adult readmission, mortality and ED measures with footnote 19: outside this CMS reporting program. These blanks are not poor results. Pediatric infection reports, family-experience measures, transfer/refusal data and behavioral-health access records are the appropriate next evidence. The state license independently records Level I pediatric trauma.

    Inspect evidence 1 β†’Inspect evidence 2 β†’

ED workload increased; the offload measure is available

Calendar-year ED encounters rose from 40,681 in 2022 to 42,686 in 2024, with 40 reported treatment stations throughout. January 2026 EMSA data show a 30.2-minute offload 90th percentile across 156 qualifying offloads, with 88.5% within 30 minutes. This describes ambulance handover; a pediatric ED visit-duration or boarding figure is not supplied by the loaded adult CMS program.

    Inspect evidence β†’

Historical penalties and community support are separately documented

The preserved CDPH extract has five distinct penalties for the matched hospital, latest issued December 2014, concerning reporting failures for pressure injury, medication error and privacy breaches. HCAI’s FY2024–25 community-benefit summary reports $240.2 million total community benefit and $5.3 million charity care. Its Medicare total duplicates the overall headline amount and needs verification against the filed plan before payer categories are interpreted.

    • Obtain post-transition RN/support-worker retention, vacancies, benefits and unit staffing data; older annual FTEs do not describe the new employment arrangement.
    • Reconcile Q2 2025’s revenue/expense discontinuity and subsequent losses with integration transactions, payroll allocation, inter-entity support and revised filings.
    • Obtain current pediatric infection outcomes, family-experience surveys, injury measures and ED boarding/transfer records outside the adult CMS program.
    • Verify the original 2025 capacity-field definition before assigning the displayed 163 to licensed, staffed, operational or functional capacity; then track usable versus licensed beds, NICU room configuration and inpatient behavioral-health opening dates during construction.
    • Verify the duplicated Medicare/community-benefit value against the original FY2024–25 plan and obtain the newer complete annual financial filing.
    • Obtain the July 2025 and April 2026 patient-protection action/correction plans, onsite follow-up evidence, hospital responses and current status; assess the November 2025 physician-related finding separately and verify current labor-case outcomes.
