A documented burn-service loss, prolonged offload times, and substantial earlier operating surpluses
420 34th Street parent hospital. HCAI 106150722, CMS 050036, CDPH ELMS 120000338 and license 120000181 refer to the same hospital through the official crosswalk. Its annual financial years end June 30.
The burn closure has a much fuller paper trail
The February 11 county letter and April 1 letter to CDPH describe nonrenewal of the Grossman agreement and attribute the decision to persistently low patient volume. The April 1 application requests complete removal of the burn center, with June 30 entered in its notice-date field. The public notice specifies seven inpatient burn beds and the outpatient burn clinic. These records establish the announced service loss and the hospitalβs stated rationale; they do not contain the underlying burn-volume or service-line financial analysis.
The February county letter names West Hills, Fresno and Los Angeles General under its former LAC+USC name. The April letter and public notice name West Hills, Fresno and UCI in Orange. The April notice gives approximate distances of 90β100, 100β110 and 140β150 miles respectively. These are the hospitalβs estimates and planned destinations, not measured transport times or evidence that receiving beds were available.
Newer quarterly results differ from annual results
Four of the five recovered quarters show calculated operating losses. JanuaryβMarch 2026 had a $10.8 million loss, a β7.5% margin, compared with a $4.9 million loss in the same quarter of 2025. Q2 2025 had a $60.2 million surplus and $67.9 million in other operating revenue; the source does not explain that revenue spike. Quarterly hospital-wide results still do not establish the burn programβs profitability or explain the closure decision.
ED encounters rose from 90,591 in 2022 to 102,229 in 2024, while ED stations remained at 36. The January 2026 ambulance-offload 90th percentile was 72.4 minutes across 1,314 qualifying offloads; 48.3% were within Kernβs 30-minute standard. These observations precede the June closure and cannot measure its consequences. CMS reports a 240-minute median ED visit and 4% leaving before being seen in their separate reporting windows.
Hospital-wide financial gains coexist with the closure rationale
Operating results rose from a $4.2 million loss in FY2021β22 to $38.2 million income in FY2022β23 and $88.5 million in FY2023β24. The last periodβs operating margin was 12.4%, with $147.1 million net income. These earlier hospital-wide results do not establish whether the burn program lost money in 2026. Productive RN hours were broadly stable across the three fiscal years, while contract RN share fell from 10.4% to 4.2%.
The licensing record narrows what can be called closed
The September 16, 2026 supplemental-service inventory contains no burn-center entry, but still lists a Center for Wound Care and Hyperbarics under outpatient wound care. Therefore the burn-center closure cannot, by itself, establish that every wound-care or hyperbaric service ended. The actual staffing and operational status of that remaining service need confirmation. CMS also classified bloodstream, urinary-catheter and C. difficile infection ratios as better than the national benchmark in its pre-closure reporting period.
Historical enforcement is now tied to the correct facility
The preserved July 2024 CDPH extract contains 19 distinct penalty records for the matched facility, after removing exact duplicate API rows. The newest issue date in that extract is 2024-02-02. The complete table retains violation dates, categories, amounts and recorded appeals. This history is not a complete count of complaints, incidents or 2024β2026 findings; original deficiency narratives and current dispositions remain to be obtained.

Representation election for a specific allied-health unit
NLRB case 31-RC-189584 records seven votes for SEIU-UHW and two against, followed by certification. The unit consists of social workers and physical therapists. It is not an RN election and does not establish RN representation.
The hospital letter to the Kern County Board names the June 30 closure and says affected employees and union representatives were notified. A dated letter does not establish the countyβs receipt date or the details of those employee notices.
Application LNC0008970 requests removal of the burn center. The letter and public notice specify seven acute burn beds and outpatient programs. April 1 to June 30 is 90 calendar days; proof of actual delivery, newspaper publication, posting and any accompanying personnel estimate remains separate evidence.
The April 7 webpage capture combines the June 30 closure notice with earlier burn-service promotional descriptions. EMSAβs April 21 directory still lists Bakersfield Memorial as an ABA-verified adult burn center, without a pediatric verification mark. The hospital had advertised pediatric burn services; advertising scope and ABA verification scope are separate facts. The directory predates closure.
The hospitalβs notices identify this as the date for ending the seven-bed inpatient burn unit and outpatient burn clinic. Actual post-closure patient transfers and receiving capacity are not supplied in the assessed documents.
The collected request seeks the May 5 Region V inquiry thread, replies, impact assessments and mitigation records for AprilβAugust 2026. It reproduces wording attributed to the May 5 inquiry. The request itself does not establish whether CDPH received replies or performed an assessment; the original thread and response are still needed.
The state inventory no longer lists a burn center for the matched facility. It retains outpatient wound care and hyperbarics. CNOβs previously published Lost Log also records an EMSA no-responsive-records response dated September 16; that response concerns EMSAβs files, not all agencies.
The hospital reported $45.2 million total community-benefit spending and $5.5 million charity care for July 2024βJune 2025. These are the filed community-benefit categories, separate from the earlier operating-finance series.
2 pages; hospital-authored letter. Low-volume rationale, three proposed destinations, employee/union notification statement. Delivery and implementation records not included.
Requests complete elimination of the burn center; FACID 120000338. June 30 appears in the notice-date field. Submission is not an approval or a closure-completion report.
One page assessed. Seven inpatient beds and outpatient clinic, three alternatives, hospital-estimated distances and payer-acceptance statement. No probable personnel reduction count or measured transfer outcomes.
All seven pages assessed. Page 3 contains closure notice plus earlier service descriptions. It is a dated pre-closure capture, not a current directory.
One-page table assessed. Bakersfield Memorial marked for ABA adult verification, not pediatric verification. Pre-closure listing; local designation and ABA verification are distinct columns.
September 10, 2026 Β· Submitted request assessed; response not obtained
Request text assessed across three pages. It seeks the original May 5 inquiry and replies, impact and mitigation records. A requester quotation is not an independently assessed agency email. Only this de-identified assessment is published.
The request response, original Region V inquiry thread, transfer agreements, burn-volume analysis and measured access effects remain outstanding. A request establishes what was sought, not what an agency found.
Evidence still needed Β· specific records and status
Obtain monthly burn admissions, burn census, staffing, program revenue and expenses, and the evaluation supporting the low-volume explanation.
Reconcile the February and April destination lists with executed transfer agreements, acceptance capacity and adult/pediatric transport protocols.
Assess actual post-closure transport times, transfer refusals, air-medical use, receiving-bed availability and continuity of outpatient burn follow-up.
Obtain the original May 5 CDPH inquiry, all replies and regional assessment/mitigation records. Confirm current wound-care and hyperbaric operations separately from the closed burn center.
Obtain original deficiency reports and correction plans for the historical enforcement records, including the 2024-issued actions. A coded adverse-event burn category does not locate an event in the burn center.
Obtain explanatory schedules for unusual quarterly revenue or expense changes and revised filings; quarterly totals do not identify the underlying accounting transactions.
The September 2026 complete annual financial release for FY2024β25 was identified. Its download returned 403 and its resource has no accessible datastore response here; those newer annual figures are not substituted for the assessed 2022β2024 series.
Site-matched OSHA injury summaries, nurse-specific injury denominators, educator staffing, vacancy and turnover records are not yet assessed. Hospital-level outcome figures cannot supply those missing workforce records.
