Mixed care obscures staffing scope; newer protection findings and SNF turnover need attention

Alameda’s combined reporting unit includes 66 acute beds and 181 skilled-nursing beds. The SNF beds are spread across Clinton Avenue (35), South Shore (26) and Park Bridge (120). Acute CMS 050211 and SNF CMS 555381 are separate records under the same hospital license.

The acute hospital has a newer patient-protection finding

The December 5, 2025 CMS complaint survey cites failures to thoroughly investigate and appropriately report abuse allegations concerning physical therapy. This acute-provider record is separate from the SNF citations and was not visible in the historical monetary-penalty extract. A March 2025 survey reported substantial compliance in the areas investigated. The December correction plan and later outcome remain to be assessed.

Inspect evidence 1 →Source 2 →

The 3.64 staffing figure is not an acute-floor staffing ratio

The FY2023–24 financial report combines 72,125 patient days, including 13,272 acute days. Most patient days are therefore outside acute care. Its 262,352 productive RN hours produce the displayed 3.64 hours per patient day. Assigning all those hours to the acute campus—or judging the acute campus against a 247-bed count—would misread the reporting unit.

Inspect evidence 1 →Inspect evidence 2 →

Reported RN labor recovered from the 2023 low

Productive RN hours were 225,730, 187,194 and 262,352 across the three financial years ending June 2022, 2023 and 2024. Nursing FTEs moved from 265 to 240 to 260. Contract RN hours fell from 97,102 to 55,299 across the period. These totals show a recovery in employed RN labor after the middle-year drop; separate acute and SNF unit allocations are still needed.

Inspect evidence 1 →Inspect evidence 2 →

The SNF has a concrete workforce signal

CMS’s August 2026 SNF release reports 86.5% total nursing turnover and 85.7% RN turnover, with a four-star staffing rating. It reports 1.46 RN hours per resident day overall and 1.18 on weekends. These are the separate 181-certified-bed SNF entity’s measures. They deserve a retention and weekend-coverage review even though the staffing rating is relatively favorable.

Inspect evidence →

Daily payroll records separate direct RN work from administration

All 90 daily PBJ records for January–March 2026 were recovered. All-role RN hours per resident day are 1.46, matching the CMS provider figure; the RN category excluding administration and the director of nursing is 1.11. Total nursing hours per resident day are 5.11, including all reported nursing roles. No contract nursing hours are reported in this quarter. The monthly and weekend breakdowns use summed paid hours divided by summed daily resident census.

Recent SNF complaint citations are now visible

The CMS SNF record carries an abuse indicator. Its citation table includes F600 resident-protection findings dated January 21 and November 25, 2025, both coded D, with correction dates February 13, 2025 and January 15, 2026. A May 21, 2026 accommodation-of-resident-needs finding lists correction June 12. The original survey narratives are needed to describe the events; the coded citation descriptions and correction dates are shown below.

The reported operating losses persist into 2026

The combined financial entity reported a $54.5 million operating loss in FY2023–24, following a $65.4 million loss the previous year. All five newer quarters show calculated operating losses; Q1 2026 was a $12.5 million loss on $29.1 million operating revenue. For this public-system hospital, the next accounting question is how system support, transfers and public financing are allocated—not just the stand-alone operating margin.

Inspect evidence 1 →Inspect evidence 2 →

Emergency workload grew; offload performance has a dated record

ED encounters rose from 16,079 in 2022 to 19,681 in 2024, while treatment stations increased from 12 to 13. The county grand jury found Alameda was the only county hospital meeting the 30-minute offload standard in its 2023 analysis. By January 2026, EMSA reported a 31.7-minute 90th-percentile offload time and 87.4% within 30 minutes. The earlier favorable finding and later borderline result describe different periods.

Inspect evidence 1 →Source 2 →

A smaller acute site is undergoing capital work

The district describes $28.9 million of seismic retrofits and $25.1 million of operational upgrades, including a proposed 18-bed short-stay SNF unit, with completion scheduled for 2028. Its 2026 bid notice covers seismic and operational projects. The district owns the buildings; Alameda Health System operates the hospital. The 18 planned beds are not added to current staffed capacity.

Patient experience adds another view of the acute hospital

In October 2024–September 2025, 69% of 319 HCAHPS respondents said nurses always communicated well; response rate was 19%. CMS classified the C. difficile infection result as better than its national benchmark, while several other infection ratios could not be calculated. The ED median visit was 153 minutes in that period, and 1% left before being seen in calendar 2024.

Operating margin = income from operations ÷ (net patient revenue + other operating revenue). Purchased services include more than contract nursing; education expenses cover more than bedside RN education. Those fields cannot establish dollars spent on travelers or nurse educators.

CDPH enforcement history · 0 distinct penalty records

Exact CDPH ELMS facility 140000011 matched to HCAI 106010735, CMS 050211 and 2070 CLINTON AVENUE, ALAMEDA. This is the July 30, 2024 extract, whose resource is titled 1997–2023 and includes 2024-issued records. It does not provide complete 2024–2026 complaint or enforcement coverage.

No acute-parent penalties matched this historical extract; this is a coverage gap, not a finding of no enforcement history. The API returned 0 matched rows; 0 exact duplicate penalty records were removed. Distinct penalties can concern the same incident. Reporting failures, privacy breaches and immediate-jeopardy penalties are separate categories.

Amounts and appeal/disposition status as recorded in the preserved extract
Issued / penalty numberViolation periodType / coded categoryInitial / final amount dueDisposition / appealed

A coded category names the event class; the original deficiency report is needed for the clinical narrative, unit location, correction plan and later outcome. A burn adverse-event category does not establish that an event occurred in the burn center.

Evidence still needed · specific records and status
  • Obtain acute versus SNF cost-center RN hours, census, vacancies, educator and relief coverage; the combined ratio cannot answer the acute staffing question.
  • Obtain the original F600 and F558 survey reports, correction evidence and current status, with the affected SNF site identified.
  • Obtain the CMS turnover measurement window and reasons for separations; investigate retention and compare daily RN coverage by SNF site.
  • Reconcile stand-alone losses with Alameda Health System allocations, transfers and public support; obtain the newer complete annual filing.
  • Confirm construction award, operational disruption plan, short-stay opening date and how the proposed unit will change staffed capacity.
  • Obtain the December 2025 acute-provider patient-rights correction plan, hospital response and subsequent inspection outcome; keep the physical-therapy setting separate from the SNF.
